Responsibilities

Job Summary
Home Health Clinical Director assumes responsibility and accountability for all aspects of the home health clinical and operational responsibilities by coordinating and facilitating care provided by multiple disciplines. The Director shall demonstrate strong general and home health clinical knowledge and leadership skills to perform as a mentor and educator. The position coordinates, manages and evaluates care provided to patients in a complex healthcare system as well as monitors and directs clinical performance. The Clinical Director actively demonstrates the ability to think at a network level to improve care for diverse populations and support the population health strategy. In collaboration with the Network Director, is accountable for overall performance for clinical and financial metrics in the home health care setting. The Director promotes an environment that supports excellence in personalized patient care and service by leading and directing implementation of clinical leadership model and collaborative practice delivery system to commit to absolute quality, clinical care and service.
  • Directs, manages and evaluates personnel assigned to the defined areas, monitors competence of staff. Leads daily operational activities. Participates in and supports the recruitment and retention strategies; provides leadership in the selection of personnel based on departmental staffing plans and personnel needs. Promotes organizational climate of decision-making, autonomy and accountability. Develops and maintains systems for clinical and management records and reports. Complies with State and Federal regulatory agencies, accreditation agencies, and other legal regulatory bodies.
  • Integrates emerging evidence based clinical practice and lead the efforts to enhance patient care. Mentors and educates staff members to develop expertise in managing complex patients in the home, assessing patient/caregiver needs along with assessing patient's health literacy in order to optimize self-care independence. Facilitates and supports clinical and other educational opportunities to provide innovative learning experiences for staff and students and create a culture of professional development.
  • Participates and oversees process improvement initiatives and monitors quality improvement data; pivots strategy when needed to obtain outcome goals.
  • Rotates after hour administrative call
  • Performs other related jobs as assigned or requested

Qualifications

Education
Bachelor's Degree in nursing from an accredited NLN or CCNE institution or Physical therapy degree from an accredited university approved by the APTA Required

Experience
3 to 5 years Home Health Experience Required

Licenses and Certifications
Arizona Nursing or Physical Therapy License Required
Driver License Required
Fingerprint Clearance Card - State Current Arizona Dept. of Health Services Fingerprint Clearance Card or shall apply and be eligible for a fingerprint clearance card within twenty (20) working days of employment Required
Basic Life Support (BLS) Required
Vehicle Insurance Required
